Adult ENT and head and neck surgery appointments can be made via physician referral or self referral.
You can refer yourself for an ENT appointment by filling in the form below. We will contact you within 2 business days with an appointment. We currently do not have a waiting list for adult ENT appointments. You are required to have a valid Alberta Health Care insurance number.
Patients, if you prefer to have a referral, please ask your family physician, dentist or audiologist to send a completed referral form (see below) or referral letter to:

You will receive a return fax to let you know that we have received the referral and that an appointment has been booked for your patient. We will also contact your patient directly with an appointment time.
